Comprehensive Care Coordination

Case Management (called Comprehensive Care Coordination in Illinois) helps senior adults determine what their specific needs and the services available to meet their needs. Every senior adult in Illinois who has services needs can receive a free comprehensive assessment.

After the assessment is completed, the Care Manager develops a Care Plan of services which helps senior adults remain as independent as possible. The cost for services, if any, is discussed and agreed to by the senior adult before they are ordered.

There are many programs available to help older people who are trying to stay in their own home with a little assistance. Making decisions about services can be overwhelming, particularly after a stay in the hospital. Decisions about aftercare most often are made rather quickly. If the right answers aren’t readily available or easily obtained, decisions about aftercare are even more difficult.

There are many public and private programs available for senior adults. Each program has its own rules and eligibility requirements. Some services have income guidelines, others do not. Many times different agencies or departments must be contacted in order to receive all the services that an older person needs. Using the Comprehensive Care Coordination assessment, the Care Manager helps to navigate these services.

The Case Management agency in Southern Illinois and Outreach Workers who are located in every county’s senior center focal point in Southern Illinois are given new information as it becomes available which they use to assist you, whether your need is due to a hospital stay or some other circumstance.
